Bristol Health Partners - Health Integration Teams

Health Integration Teams are set up to improve care in Bristol, North Somerset and South Gloucestershire (BNSSG). They work across our partnership to tackle some of the critical health and care challenges people in our region face.

Each Health Integration Team (HIT) is unique, but they all share a common goal to improve care in the BNSSG region.

Experts in research, health and care services, and public health work alongside patients, carers and service users to make sure that the best evidence is found and put into practice. By focusing on key areas and bringing together the right people from across their partnership, HITs help people to live healthier lives, prevent illness earlier, and better integrate health and care across Bristol, North Somerset and South Gloucestershire.
